Comment:

Your patient has had an adequate trial of at least one antipsychotic from the new generation group. The next questions will assess if the patient has also had an adequate trial of a "standard typical neuroleptic" (or equivalent). If the patient has had such a trial, the patient will meet minimal criteria for a trial of clozapine. We will not necessarily recommend clozapine yet, however.

If the patient has not had a trial of a standard, typical neuroleptic, you will not be encouraged to try one yet. It will be suggested that you try a second new generation drug. You need to have tried at least two antipsychotics before clozapine, and we think one of these should be from each of these two groups, but our preference is actually for three trials before clozapine, with two from the newer generation group.
